1. Record the site and exact location.
2. Choose a useful issue category.
3. Add a clear photo of the condition.
4. Write what happened and what needs attention.
5. Assign a responsible person and explain the next step.
6. Update progress when work changes.
7. Attach completion evidence before review.
8. Review the outcome and include it in the site report.

For urgent or dangerous situations, follow your organisation's emergency procedures first. A software update does not replace an emergency response.
